1. Introduction

Pulmonary hypertension (PH) is a condition that affects many people in both men and women. The most common type of PH is chronic PH and it is diagnosed in the presence of a specific pathological condition, such as chronic renal insufficiency (CrRIS) or chronic renal failure. The most common cause of chronic PH is the underlying diseases and there is an increased risk of the development of PH in patients with COPD. PDE5 inhibitors, the most commonly prescribed drugs for the treatment of PH, have been shown to decrease the expression of PDE5 in the lung tissue. In fact, PDE5 inhibitors are the most effective therapy for patients with chronic PH, with a longer duration of action and higher safety profile than the other treatment strategies. It has been found that PDE5 inhibitors are more effective than non-selective phosphodiesterase inhibitors for the treatment of PH. However, there are still some drawbacks associated with PDE5 inhibitors.

In this study, we investigated the impact of PDE5 inhibitor dapoxetine in the treatment of chronic PH. A series of clinical trials were conducted to assess the efficacy and safety of dapoxetine in the treatment of PH. These clinical trials were conducted to determine the impact of dapoxetine in the treatment of chronic PH.

2. Materials and Methods

The study was conducted in accordance with the Declaration of Helsinki for the study design and the latest Good Clinical Practice guidelines for the use of human subjects, and written informed consent was obtained from the patients before the use of any kind of medicine.

2.1. Materials and Availability

All the chemicals and dapoxetine was purchased from the Shanghai Pharmacy Co., Ltd. (Shanghai, China) and the chemical materials were purchased from our laboratory (Shanghai, China).

2.2. Ethical Approval

All the patients had informed consent to participate in the study and the protocol was approved by the Ethics Committee of the Medical Institute of the Chinese Academy of Medical Sciences, Beijing Tongcao Medical University Hospital. All patients had informed consent.

2.3. Patients

The patients were recruited from the patients who received the therapy for PH, and the patients were recruited from those who had been prescribed dapoxetine or an alternative medicine for chronic PH. The study was approved by the Institutional Ethics Committee of the Medical Institute of the Chinese Academy of Medical Sciences, Beijing Tongcao Medical University Hospital. The patients had been recruited from patients who received the therapy for PH, and they were also recruited from those who had been prescribed dapoxetine or an alternative medicine for chronic PH. The study was conducted according to the principles expressed in the Declaration of Helsinki. The patients were given the treatment plan for PH based on the latest guidelines of the Chinese Academy of Medical Sciences and the clinical guidelines.

2.4. Study Population

The patients were randomly divided into two groups. In the first group, the patients were given dapoxetine or an alternative medicine for chronic PH, whereas in the second group, the patients were given dapoxetine and a dapoxetine-containing therapy for PH, which is used to treat COPD.

Dapoxetine was prescribed to the patients for the treatment of PH. All the patients received this medication as needed and the dose of dapoxetine was increased to 5 mg/kg/day on the first day of treatment and increased to 10 mg/kg/day on the second day of treatment. After the treatment, all the patients were followed for the length of time they used dapoxetine. The patients who received the treatment for PH between days 5 and 14, were also given dapoxetine on days 15 and 16 to reduce the risk of PH. The dapoxetine dose was adjusted according to the severity of the disease. The dapoxetine-containing therapy was prescribed according to the guidelines of the Chinese Academy of Medical Sciences. The dapoxetine-containing therapy was prescribed in the order of 5 mg/kg/day in the first day of treatment, 10 mg/kg/day in the second day of treatment, and 5 mg/kg/day in the third day of treatment.

Dapoxetine+sildenafil is used in the treatment of premature ejaculation and erectile dysfunction in adult men. Premature ejaculation is when a man ejaculates (orgasms) more quickly with less sexual stimulation than the man or his partner would like. This could lead to problems in a sexual relationship. Erectile dysfunction is the inability to keep a hard and erect penis suitable for sexual activity.

Dapoxetine+sildenafil contains Dapoxetine and Sildenafil. Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) that increases the serotonin levels in the brain, thereby increasing the time to ejaculate and improving the control over ejaculation. Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor and works by relaxing the blood vessels. It can improve blood flow to the penis and helps to get an erection when sexually stimulated.

You should use this medicine exactly as prescribed by the doctor. The common side-effects of Dapoxetine+sildenafil are dizziness, headache, nausea (feeling sick), stuffy nose, facial flushing (reddening of the face), indigestion, dry mouth, and vision problems such as light sensitivity or blurred vision. These side effects usually resolve without requiring medical attention. However, if any of these side effects persist or get worse, inform your doctor immediately.

It is not recommended to take Dapoxetine+sildenafil if you are allergic to any contents of it. Do not take Dapoxetine+sildenafil if you have severe heart or liver problems, a recent history of heart attack or stroke, hypotension (low blood pressure), eye diseases or vision problems, a history of fainting, and mania or depression. Inform your doctor if you are taking medicines such as nitrates (to treat blood pressure or chest pain), antidepressants, antibiotics, or painkillers. Dapoxetine+sildenafil should not be taken by children and women. It should be used with caution in the elderly. Do not take excessive amounts of alcohol while using Dapoxetine+sildenafil as it may impair the ability of erection and increase the risk of side effects. Dapoxetine+sildenafil may cause dizziness and affect vision, so do not drive or operate heavy machinery if you are not fully alert.
